Unilab's high throughput laboratory in Huvudsta – the first fully digital laboratory within the Unilab Group - part one

 

Unilab's high throughput laboratory in Huvudsta is the newest and most modern pathological lab within Unilabs in Europe. It is built on best practice with a consistent lean thinking that follows our processes. In parallel, we have worked with the introduction of a new LIS in the business and this has meant that all pathology labs in Sweden today are completely paperless in all process steps. Our new LIS in combination with the introduction of digital scanners means that Huvudsta took the step to become the first fully digital laboratory within the Unilab Group.

All diagnostics are done here 100% digitally with distance pathologists in our diagnostic network. Thanks to the experience from the establishment of Huvudsta, today all our Swedish histopathology labs are 100% digital from referral in, via the work in the lab, diagnostics and answers to the customer and participation in MDK (Multidisciplinary conferences). Our digital journey has brought a unique opportunity for our employed pathologists to alternate telework with work on site, which has been an advantage not least during the pandemic.

 

Unilab's Clinical Pathology Laboratory in Huvudsta – part of Sweden's cancer care

In April 2019, Unilabs opened its high throughput lab in Huvudsta, Stockholm. We are a modern, completely digital pathology lab that handles 270 cases every day, the sole purpose of the work we do is to identify changes in e.g. skin, intestines, colon, ureters or gynecological samples.

Samples taken by your doctor are sent to us at Unilabs for diagnosis. We analyze tissue samples to see if there are abnormalities such as cancer or inflammation present.

At a Clinical Pathology Lab, the sample is received, registered on our computer system, analyzed macroscopically, i.e. a Biomedical Technician looks at it and cuts the sample into parts (formally known as dissection) in preparation for diagnosis. The sample is then dehydrated and embedded in paraffin, cut into a four µm thick sections and placed on a slide for routine staining (HtX), special staining or by immunohistochemical methodology.

When the slides are stained and fixed, they are then quality checked and scanned into our digital scanners. The digital scanners act as a microscope, which allow for pathologists to make diagnosis from a digital image on their computer screens without a need for a traditional microscope. The scanned images are allocated to their patient profile (known as a referal) with the digital system. When the case is complete, it is sent to a pathologist who looks at the digital slides makes a diagnosis from the ultra high definition images. When the diagnosis is reported, an answer is sent to the referring physician.

Unilab's Clinical Pathology Lab is part of the region's healthcare and we have an agreement with a turnaround time of ten days for standard and routine samples. We handle emergency cases in three days and SVF (Standardiserat Vård Flöde) is handled in five to seven days depending on the type of tissue.

 

Step one – Registration

The first thing we do is sort all the samples that come to us in boxes. Before they can be registered, we check that both the tissue and the referral are available. 90% of all referrals we handle are received in electronic form to our digital laboratory information system (LIS). Paper referrals are registered manually, and the referral is scanned as a copy to the case. If a referral is missing or if sample tubes from the same patient cannot be differentiated or are not clearly marked, the customer is contacted in order to provide the information needed to rectify the discrepancy. At the point of registration the tests are linked to the received referral, the information on the tests is identical with the doctors request on the referral. The case is given a new digital ID number and all samples belonging to the same case are labeled with this number and can then identified at each station in the process.

We are a completely digital lab, which to allows full traceability of all components at every stage of the processing, having a paperless system allows for us to give the highest quality service.

Parallellt har vi arbetat med införande av ett nytt LIS i verksamheten och det har medfört att alla patologilab i Sverige idag är helt papperslösa i alla processteg. Unilabs Kliniska patologilaboratorium – en del av Sveriges cancervård

I april 2019 öppnade Unilabs sitt volymlaboratorium i Huvudsta, Stockholm. Vi är ett modernt och helt digitalt patologilab som varje dag hanterar omkring 270 remisser

Vävnadsprov som du lämnat hos din läkare skickas till oss på Unilabs för diagnostik. Vi analyserar provet för att se om allt står rätt till eller om det finns någon sjukdomsbild i din kropp som cancer eller inflammation i t. ex. hud, tarm, urinledare eller livmoderhals.

På vårt patologilab tas provet emot, ankomstregistreras, analyseras makroskopiskt, d. v. s en Biomedicinsk analytiker tittar på det och skär provet i lämpliga delar som gör att det blir lättare att analysera. Därefter dehydreras provet och bäddas in i paraffin, snittas i ett fyra µm tjockt snitt som läggs på ett objektglas som sedan färgas med rutinfärgning (HtX), specialfärgning eller med immunohistokemisk metodik.

När glasen är färgade och monterade, kvalitetskontrolleras och skannas de i våra digitala mikroskop s. k. skanners. De skannade bilderna kopplas till respektive remiss och när fallet är komplett skickas det till en patolog som tittar på de digitala glasen och diagnosticerar. När diagnos skrivits, går det ut ett svar till den läkare som har skickat in provet.

Unilabs Kliniska patologilab är del av regionens hälso- och sjukvård och vi har ett avtal med svarstid på tio dagar för standardprover. Akuta fall hanterar vi på tre dagar och SVF-märkta prov (Standardiserat Vård Flöde) hanteras på fem till sju dagar beroende på organområde.

 

Det första steget – Registrering

Alla prov kommer in till Unilabs i plomberade lådor som packas upp och sorteras. Vid sorteringen så stämmer vi av att både prov och remiss finns inskickat. 90% av alla remisser vi hanterar inkommer i elektronisk form till vårt digitala journalsystem. De remisser som kommer in i pappersform registreras manuellt och remissen skannas in som bilaga till underlaget. Om en remiss saknas eller om de rör som tillhör samma patient, inte går att särskilja eller inte är särmärkta, behöver vi kontakta kunden för att få hjälp med att komplettera fallet innan vi kan registrera in det i vårt system.

Vid själva registreringen då prov kopplas till inkommen remiss, stäms informationen på proven av med det läkaren skrivit i remissen. Fallet får ett nytt digitalt ID-nummer och alla prov som hör till samma fall etiketteras med detta nummer som sedan kan spåras/följas vid varje station i hanteringskedjan, processen.

Vi är ett helt digitalt laboratorium, vilket bidrar till en spårbarhet av alla ingående delar ned till mikroskopglas och vi är helt papperslösa genom hela processen.
